Healthcare Navigator Role

We are seeking a skilled and compassionate Healthcare Navigator to join our team. The ideal candidate will have a strong understanding of healthcare systems and be able to navigate consumers through various services and support.

  • This is a full-time opportunity with flexible work arrangements, including the option for a Rostered Day Off each month.
  • The service operates seven days a week, and candidates should be willing to work on a rostered basis, including weekends and outside of normal business hours.
Key Responsibilities:
  1. Navigate consumers through various healthcare services and support.
  2. Provide care coordination and referral support to consumers.
  3. Collaborate with multidisciplinary mental health teams to provide holistic and collaborative care.
Requirements:
  • Tertiary qualification in a relevant discipline, or extensive experience in mental health, health, community services or social sciences.
  • Demonstrated understanding of health and wellbeing needs of consumers with mental health and/or substance use problems.
  • Demonstrated ability to engage with consumers and their families or carers to provide care navigation and referral support.
