We are seeking an enthusiastic individual to join our team as a Supports Coordinator.

About the Role

This is a full-time opportunity working within business hours, Monday to Friday. As a Supports Coordinator, you will assist participants of the NDIS to connect to local services and supports.

Main Responsibilities
  • Liaise with clients and families to coordinate their NDIS plan supports scheduling and claiming.
  • Collaborate with clients and families to build on their NDIS plan knowledge and available support streams within the local area based on their choices.
  • Identify potential under/over servicing for clients and take action if goals or supports are not being delivered as per service agreements.
  • Maintain a thorough understanding of the NDIS price guide and support categories.

  • Strong problem-solving skills.
  • Ability to maintain confidentiality and handle sensitive information.
