Job Description

The Community Access Unit Clinician will play a vital role in processing referrals from internal and external sources for ambulatory care and community services. This includes refining clinical skills, maintaining patient-centred care, and providing a holistic view of the care continuum.

This position offers the opportunity to expand knowledge on the broad suite of services provided by Eastern Health's ambulatory care and community services, as well as develop or refine clinical triage skills in a supportive environment.

Key Responsibilities
  • Process referrals from internal and external sources for ambulatory care and community services
  • Refine clinical skills while maintaining patient-centred care
  • Provide a holistic view of the care continuum, from receipt of referral to clinic acceptance
Requirements
  • Current clinical qualification and registration as a Registered Nurse or in an Allied Health discipline
  • Knowledge of patient pathways through the acute, subacute, ambulatory, and community service
  • Highly developed interpersonal and leadership experience
