As a highly organised and motivated Registered Nurse, you will play a vital part in our Triple I Central Intake team – a key entry point into care across our Primary and Community Health services.
This is an exciting opportunity for someone who thrives in varied environments, values collaboration, and takes pride in providing timely, accurate care and support that contributes directly to better health outcomes for patients.
Key Responsibilities- You will be part of a dedicated team supporting patient access to essential health services.
- Work closely with clinicians, enrolled nurses, and stakeholders to ensure seamless care coordination.
- Liaise with clients, carers, service providers, and referrers across diverse cultural and socioeconomic backgrounds.
- Help coordinate care and support for clients and their networks across the community.
- Current Authority to Practice as a Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA).
- Minimum five years of clinical experience in acute or community care settings, with well-developed clinical knowledge and ability to provide safe, high-quality, patient-centred care.
- Excellent communication and interpersonal skills, with proven ability to apply critical thinking and clinical judgement in managing complex patient care.
- Australian C-Class driver's license (including P1 or P2) required.
