Key Responsibilities:

  • Deliver High-Quality Medical Care to Clients in Their Homes and Other Settings
  • Develop and Implement Effective Diabetes Management Plans
  • Provide Dialysis Services as Required
  • Manage Medications and Treatments to Optimize Patient Outcomes
  • Palliative Care to Improve Quality of Life for Clients
  • Perform Phlebotomy Procedures as Needed
  • Offer Wound Care Services, Including Negative Pressure Therapy and Complex Wound Treatment

Requirements:

  • Valid LPN License in Province of Practice
  • Previous Experience in Community Health an Asset
  • Valid Driver's License and Reliable Vehicle Access Required
  • Current and Satisfactory Criminal Records Check, Vulnerable Sector Search, and Child Abuse Registry Check (If Applicable)
  • Able to Wear Personal Protective Equipment
