In this role, you will coordinate personal care for residents in accordance with care plans which meet their individual needs and preferences.
- Deliver high quality, person centered care to residents and families
- Provide personal care to residents with complex needs
- Support nursing staff regarding assessment, care planning processes, documentation and evaluation of care outcomes
- Monitor the cleanliness, safety, environment and promptly report issues of concern
This is the ideal role for you if you have:
- Certificate III in Individual Support, or equivalent (Aged Care)
- Experience in residential aged care/home care
- Experience with dementia care and palliative care support
- Demonstrated abilities to support residents in a fast paced environment
- A current Working With Children Check card
- Current and ongoing flu vaccinations
- Australian working rights
- Ability to pass a national police clearance
