About the Role

We are seeking a skilled and compassionate Discharge Planner to join our team at John Fawkner Private Hospital. As a key member of our multidisciplinary team, you will play a vital role in ensuring seamless patient flow and effective bed utilization.

Key Responsibilities
  • Develop and implement optimal discharge plans in line with hospital strategic direction and operational processes.
  • Liaise with Unit Managers, nursing staff, allied health professionals, and medical practitioners to coordinate patient discharge plans.
  • Collaborate with external service providers to ensure patients receive ongoing care and support post-discharge.
  • Provide education and support to patients and families regarding health and nursing care.
  • Conduct early screening of inpatients with complex care needs to facilitate timely discharge.
Requirements
  • 2-4 years of experience as a Registered Nurse in an acute setting.
  • Excellent interpersonal and time management skills with the ability to work within a multidisciplinary team.
  • Ability to generate referrals to relevant organizations.
  • Capacity to support and liaise with respective NUMs and departments.
  • Ability to problem-solve in a fast-paced environment.
