About the Role

The Community Registered Nurse will support our Home Care program based out of a Ballarat office, servicing clients in the region.

  1. Conduct in-home clinical assessments, reviews and care needs for East and Outer Eastern suburb clients
  2. Provide clinical advice and support to clients, referring them to services as needed
  3. Lead and supervise Home Care staff and provide collaborative leadership
  4. Work with local allied health professionals to facilitate responsive care in clients' homes
  5. Offer health education and promotion, including social support groups and carer education
  6. Promote a harmonious team approach by setting a positive example in coordinating daily routines and adhering to safe practice measures in medication management and achieving clinical goals

Key Responsibilities:

  • Bachelor of Nursing or equivalent qualification with current AHPRA Div 1 Nursing registration (required)
  • Minimum 3-5 years experience as a Registered Nurse in Community, Commonwealth Home Support or District Nursing (required)
  • Team player, flexible in your approach to work with a positive outlook
  • Endorsed for Continence Assessments and Prescription (highly desirable)
  • Experience in actively visiting clients in their homes
  • Experience with evaluating and implementing care support plans
  • Familiarity with the My Aged Care portal
  • Proficiency in decision making, critical analysis and on the spot problem solving
  • Demonstrated abilities to delegate and prioritise in a fast paced environment with exemplary communication skills in a team and one on one interactions

  • A current Working With Children Check card
  • Current and ongoing flu vaccinations
  • Australian working rights
  • Ability to pass a national police clearance and NDIS Clearance
  • Full driver's licence and comprehensive car insurance and car registration in your name
