Dedicated Home Care Professional Wanted

As a Registered Nurse in our community care team, you will provide high-quality, person-centered care to clients in their own homes. You will be responsible for delivering clinical in-home services, including care planning, medication management, and wound care.
Key Responsibilities:- Providing clinical in-home services, focusing on independent living and community support.
- Supervising non-clinical team members to ensure seamless service delivery.
- Handling client intake, care planning, and reporting with accuracy and attention to detail.
- Conducting comprehensive assessments with a focus on Consumer Directed Care principles.
- Coordinating quality care tailored to individual client needs and goals.
- Managing documentation to meet regulatory requirements.
- Current registration as an Enrolled Nurse (or higher qualification) with AHPRA.
- First Aid & CPR certification.
- Australian Police Check.
- Professional Indemnity Insurance.
- Experience in community care assessment and planning.
- Valid driver's license and reliable vehicle.
- Smart mobile device (iPhone/Android).
- Strong passion for delivering high-quality, client-focused community care.
- Cert IV in Training and Assessment (highly desirable).
- Excellent communication skills, with the ability to work independently and collaboratively within teams.
- Strong time management and organizational skills.
- Commitment to ongoing professional development and staying up-to-date with industry best practices.
