Role Overview

We are seeking a proactive, compassionate and forward-thinking professional to join our team and play a vital role in delivering high-quality, person-centred care across the Northern Rivers region.

Key Responsibilities:

  • Coordinate care services under the Support at Home framework ensuring clients are empowered to make informed choices and maintain independence.
  • Prepare and review budgets with clients to align with their care objectives.
  • Manage a caseload conducting regular assessments and reviews.
  • Build and maintain strong, trusting relationships with clients and families.
  • Coordinate services across allied health, nursing and home support.

Required Skills & Qualifications:

  • Previous experience in aged care case management HCP/CHSP preferred.
  • Background in community or aged care service coordination.
  • Excellent planning communication and interpersonal skills.
  • Ability to work independently while contributing to a multidisciplinary team.
  • A valid Australian driver's licence and clean driving record.
  • Eligibility to obtain a National Police Check and NDIS Worker Screening Check.
