About the Role
We are seeking an experienced Therapeutic Care Case Manager to join our team at Anglicare. As a Case Manager, you will provide tailored support to vulnerable individuals and families, collaborating with clients, families, carers, and service providers to assess needs, develop care plans, and monitor progress.
Key Responsibilities
- Provide individualized support to adolescents and families with trauma, promoting their well-being and resilience.
- Develop and implement comprehensive care plans, working closely with clients, families, and service providers to ensure effective support.
- Conduct regular assessments and reviews to monitor progress, making adjustments to care plans as needed.
- Collaborate with internal and external stakeholders to ensure seamless service delivery and optimal outcomes.
Requirements
- Diploma in Community Services or a graduate degree in psychology, social work, or a related field.
- Experience supporting adolescents and families with trauma, with a strong understanding of inter-generational trauma and relevant legislation.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and stakeholders.
