Occupational Therapist/OT Role

We are seeking a highly skilled Occupational Therapist/OT to provide comprehensive care services to patients in their homes.

About the Job:

The Occupational Therapist/OT will be responsible for assessing and evaluating patient care needs, developing treatment plans, and implementing interventions to enhance patient well-being.

Key Responsibilities:
  • Home-Based Care Delivery: Provide occupational therapy services to patients in their homes, utilizing evidence-based techniques and collaborating with the healthcare team to ensure seamless care.
  • Patient Assessment and Planning: Evaluate patient functional status, occupational therapy needs, and develop treatment plans in accordance with state laws and CMS documentation guidelines.
  • Progress Monitoring and Reporting: Observe, record, and report patient response to treatment and changes in condition to the nurse supervisor and physician.
  • Caregiver Education and Support: Instruct and inform patients, family members, and caregivers regarding the plan of care, functional limitations, and progress towards goals.
  • Travel and Documentation: Visit patients within the service area and nearby LHC agencies as needed, completing progress notes and submitting them according to LHC policy.
  • Treatment Plan Development and Implementation: Develop and implement the OT plan of care, reporting outcomes of evaluation, goals, and projected frequency of care to the nurse supervisor within 24 hours.
  • Collaboration and Leadership: Participate in staff conferences and committees as requested by the agency, supervising home health aides and OTAs per LHC policy and state regulation.
  • Clinical Knowledge and Skills:
  • Maintain up-to-date clinical knowledge and skills, participate in weekend rotations, and access education to enhance professional practice.
