Social Links Program Assistant

At ACH Group, we are committed to delivering exceptional social experiences that enable customers to live fulfilling lives. Our Home Care Community program is seeking a dedicated and passionate individual to join our team as a Social Links Program Assistant.

About the Role:

We are looking for someone who can provide time-limited individual planning, support, and coaching to customers around their social goals. This involves identifying opportunities for customers to participate in activities that match their interests, promoting our programs, and working collaboratively with colleagues to achieve customer outcomes.

Main Responsibilities:

  • Develop and implement individual plans to achieve customer social goals
  • Provide support and coaching to customers to help them access social activities and events
  • Promote our Social Links programs to customers and stakeholders
  • Work collaboratively with colleagues to plan and deliver customer participation initiatives

Requirements:

  • Candidate must have strong customer service skills and be able to work effectively in a team environment
  • Strong documentation skills with intermediate computer literacy
  • Able to motivate and engage individuals and enhance their abilities, skills, and interests
  • Excellent interpersonal and communication skills
  • Tertiary level education in health, community, or other relevant area or Certificate IV in Community Services/ Aged Care/ Service Coordination or working towards
