About the Opportunity

We currently have an exciting opportunity to join the Metropolitan Youth Health - West team as Social Worker,where you will be responsible for:

  • Engaging and supporting vulnerable youth by Building strong relationships with disadvantaged young people, helping them access and navigate specialised primary health care and support services.
  • Delivering Client-Centred case management by Providing high-quality, multidisciplinary care and psychosocial support within a case management framework aligned to the Metropolitan Youth Health Model of Care.
  • Collaborating across services and working closely with young people, families, health professionals, and community agencies to improve access, outcomes, and coordinated care for youth.
  • Participating in research, evaluation, and the development of policies and procedures to enhance service quality and respond to the evolving needs of young people.
  • Leading professional development by providing supervision and mentoring to team members and students, fostering ongoing learning and a positive, culturally sensitive work environment.

About You

To be successful in this role, you will have:

  • Degree or equivalent enabling full membership with the Australian Association of Social Workers.
  • Proven ability to build collaborative relationships, manage conflict, and communicate effectively across diverse populations and multidisciplinary teams.
  • Skilled in assessment, case planning, psychosocial support, referral, advocacy, and follow-up with young people facing complex needs.
  • Experience planning, facilitating, and evaluating group projects addressing complex youth issues.
  • Demonstrated ability to supervise, mentor, and support staff and students.
  • Strong understanding of adolescent development, youth health issues, social determinants of health, mandatory notifications, duty of care, and culturally sensitive service delivery particularly for Aboriginal and Torres Strait Islander young peoples and those under guardianship.

About MYH:

  • MYH works on prevention, early intervention and youth specialist health response measures regarding adverse childhood events (ACE) and their impact on young people's health, including speciality programs for young parents
  • This position is located at the clinical health service location of MYH West (which is also the home base for the MYH Young Parents. Program), where multiple support programs run, including the Talking Realities FLO Program and various grant funded Young Parents support Programs.
  • MYH programs at this site, focus on building and strengthening safe and positive relationships for all young people accessing our health services but with special focus on young parents and their children.
  • Building these protective factors improve children and young people's resilience and reduce the impacts of ACE's on their future health and disrupting intergenerational cycles.
