Senior Care Manager Opportunity

We are seeking a skilled and compassionate Senior Care Manager to join our Home Care team. As the key link between our clients and their care, you will ensure older Australians receive the right support, at the right time, in the way that best suits them.

Key Responsibilities

  1. Assess Client Needs: Conduct initial and ongoing assessments to understand physical, emotional, and social needs, identifying risks and tailoring support plans with clients and families.
  2. Care Planning: Develop, review, and update person-centered care plans aligned with client goals and funding.
  3. Service Coordination: Organise in-home supports and liaise with external providers to ensure timely and quality care.
  4. Advocacy and Education: Support clients to understand their funding, choices, and rights, and advocate for access to appropriate services.
  5. Budget Oversight: Monitor individual budgets and guide clients on effective use of funds.
  6. Relationship Building: Build strong connections with clients, carers, and providers, supporting smooth transitions and ongoing care.
  7. Compliance and Safety: Maintain accurate documentation, uphold aged care standards, and proactively manage risks.

Requirements

  • Strong communication skills, highly organised with advanced problem-solving skills.
  • A strong understanding of Home Care Packages, Commonwealth Home Support Program, and related funding models.
  • Currently holds a Certificate III in Individual Support or similar qualification.
  • Currently holds a Medication competency certification.
  • First Aid and CPR qualifications are essential for this role.
  • A current driver's license and a reliable, registered, and comprehensively insured vehicle suitable for work purposes are necessary.
  • Hold valid Australian Work Rights – unfortunately, we are not currently able to sponsor.
