Job Summary: We are seeking a compassionate and experienced Nurse Practitioner with a background in hospital aged care, including dementia care, medical, and rehabilitation settings. The successful candidate will provide high-quality care to elderly patients, ensuring their physical and emotional needs are met in a supportive environment.
About The Role:
We have multiple contracts available across healthcare services in various locations, ranging from 4-12 weeks with potential to extend. Duties for this contract include:
- Deliver comprehensive nursing care to elderly patients in hospital and aged care settings.
- Manage and assess patient care plans, including those related to dementia and rehabilitation.
- Administer medications and treatments as prescribed and monitor patient responses.
- Collaborate with multidisciplinary teams to develop and implement individualized care plans.
- Provide emotional support and education to patients and their families.
- Ensure adherence to all safety, infection control, and regulatory standards.
Key Requirements:
- Experience in hospital aged care, including dementia care and rehabilitation.
- Strong understanding of medical and rehabilitation needs specific to the elderly.
- Excellent communication and interpersonal skills.
- Ability to work both independently and as part of a team.
- AHPRA Registration (must be current).
- Australian National Police Check.
- Australian Immunisation History.
- Additional Mandatory Trainings.
