Job Title

A Licensed Practical Nurse is required to provide nursing care and support to residents in a senior living community.

Responsibilities
  • Administer medications as prescribed by a physician
  • Monitor residents' health and wellbeing
  • Provide leadership support and assistance with resident care and services
  • Assist with completing the clinical sections of monthly wellness visits and PSP to accurately reflect the resident monthly status
  • Inform the Wellness Director of any changes in medication or service level that may result in a higher tier level or potential move-out
  • Assist with obtaining weights and vital signs monthly for each resident prior to completing monthly wellness health updates
  • Assist with direct resident care as needed
Requirements
  • Licensed Practical Nurse (LPN) licensure
  • Minimum one year experience working with elders or disabled individuals
  • Demonstrated leadership capabilities
  • Knowledge and experience in Assisted Living, Dementia care, or Skilled Nursing preferred
  • Ability to work weekends, evenings, and flexible hours
