About the Role

We are seeking a highly skilled Physiotherapist to join our team at Southern Cross Care, a leading provider of aged care services. As a Physiotherapist in our residential care setting, you will play a vital role in delivering high-quality care to our residents.

Key Responsibilities
  • Conduct comprehensive assessments and develop individualized care plans to optimize the mobility, pain management, and recovery of our residents.
  • Collaborate with our multidisciplinary team to ensure seamless care delivery and effective communication.
  • Participate in face-to-face handovers with nursing staff to ensure resident needs are met and therapy goals are achieved.
  • Contribute to case conferencing and problem-solving to address complex care needs.
  • Support residents on proactive reablement pathways and identify deviations from these pathways early.
  • Provide guidance and mentorship to students on placements and engage in ongoing professional development to enhance your skills and knowledge.
About You

We are looking for a Physiotherapist who is passionate about delivering high-quality care to older adults. To be successful in this role, you will have:

  • A Bachelor or Master degree in Physiotherapy and current registration with AHPRA.
  • Knowledge of residential aged care practice, including understanding and experience of AN-ACC.
  • Experience working from a Healthy Ageing approach.
  • Demonstrated commitment to meeting the needs of internal and external clients.
  • Strong organizational and time management skills.
  • Excellent interpersonal skills, including the ability to build and maintain strong relationships with stakeholders.
  • Proficient computer skills, including the ability to use the Google Suite and in-house databases.
  • A basic knowledge of the requirements of the Aged Care Standards applicable to the role.
  • The capacity to apply for a provider number and understanding of the aged care system.
