Physiotherapist Role

We are seeking a skilled and experienced Physiotherapist to join our Home and Community Care team.

The successful candidate will work in a supportive environment that encourages initiative, using evidence-based interventions and a reablement approach to improve the independence of customers.

This is an autonomous role that requires travel in the North Metropolitan area providing care to customers in the community and occasional time on site at facilities.

Key Responsibilities:
  • Conduct comprehensive assessments and develop personalized care plans to meet each client's unique needs
  • Design exercise programs to keep clients motivated and engaged to optimize their function and independence
  • Assess and provide equipment solutions to support client autonomy and well-being
  • Communicate effectively with clients, families, and caregivers to achieve seamless care coordination and accurate progress tracking
Requirements:
  • Current FULL registration with AHPRA as a Physiotherapist (essential)
  • Ability to relate to elderly customers with empathy whilst working effectively with other health professionals and service providers
  • Excellent interpersonal and communication skills
  • Proven ability to assess, plan, implement, and evaluate quality care
  • Current Australian driver's licence and reliable vehicle (both essential)
