About the Role:
As a vital member of our healthcare team, you will be instrumental in ensuring effective discharge planning for patients. This role offers a part-time opportunity to work flexibly across weekdays, contributing to a supportive and collaborative environment.
Key Responsibilities:
- Oversee comprehensive discharge planning aligned with the hospital's operational strategies and budgetary guidelines.
- Facilitate smooth patient transitions, optimizing bed utilization and managing care episodes within a dynamic team framework.
- Collaborate with nursing staff, unit managers, allied health professionals, and medical practitioners to coordinate effective discharge plans.
- Engage with external service providers to address ongoing care needs for patients post-discharge.
- Provide essential information and options to patients and their families regarding health and nursing care.
- Conduct early assessments of inpatients with complex care requirements to promote timely discharges.
- Ensure that all discharge plan documentation is accurately recorded and accessible to clinical staff.
Selection Criteria:
- A minimum of 2 to 4 years of experience as a Registered Nurse in an acute care setting.
- Strong interpersonal and time management skills, with the ability to thrive in a multidisciplinary team.
- Proficient in generating referrals to appropriate organizations.
- Capable of supporting and liaising with relevant nursing unit managers and departments.
- Adept at problem-solving in a fast-paced healthcare environment.
- Ability to work independently as well as collaboratively within a team.
