Community Mental Health Practitioner

Deliver high-quality mental health services to individuals within a community setting. Play a vital role in supporting patients on their journey towards recovery and improved well-being.
Implement the Treatment and Care Plan developed by the multidisciplinary team and the consumer. Work collaboratively with consumers and families/carers, liaising closely with government and non-government organisations to maximise wellbeing, stabilisation of mental health, rehabilitation, and building functional capacity for self-management.
Key Skills and Qualifications- Comprehensive assessment skills (including risk) to inform case conceptualisation and a treatment plan.
- Ability to implement and evaluate appropriate interventions.
- Ability to work collaboratively with all key stakeholders to design individual service plans and coordinate comprehensive service provision including rehabilitation programs from a psychosocial perspective, for consumers who have enduring and complex needs.
This is a term vacancy for 12 months, with the potential of extension, including ongoing appointment, at the end of that period.
