About the Role

We are seeking an experienced hospital pharmacist to join our Formulary Team, where you will play a key role in managing the medication formulary and working closely with other pharmacy and medical teams.

As a member of our team, you will be responsible for reviewing single patient use requests and formulary applications, implementing the pharmacy department procurement plan, and managing medication shortages.

You will work closely with the pharmacy leadership team, team leaders, and procurement teams to ensure the effective management of medication logistics and formulary management.

Key Responsibilities
  • Manage the medication formulary, including review of single patient use requests and formulary applications
  • Implement the pharmacy department procurement plan
  • Manage medication shortages
  • Work closely with other pharmacy and medical teams to ensure effective medication logistics and formulary management
Requirements

To be successful in this role, you will need:

  • Significant hospital pharmacy experience
  • A genuine interest in medication formulary management
  • Highly professional and able to lead by example
  • Relevant postgraduate qualifications, such as master of clinical pharmacy or equivalent
  • Current AHPRA registration as a Pharmacist
