We are seeking a Behaviour Support Practitioner to provide positive behaviour support to NDIS participants.
\This role involves working with a diverse client population and mixed caseloads, inspiring, empowering and supporting our team and clients to reach their full potential.
\As a key member of our multidisciplinary team, you will play a vital role in providing evidence-based support plans within a complex support space.
\- Assist NDIS participants, their families and/or carers in identifying needs through assessment and person-centred planning
- Evaluate, develop and deliver evidence-based support plans within a complex support space
- Develop meaningful goals and practical strategies using a positive behaviour support framework
- Ensure individual plans reflect NDIS participants' goals, passions and strengths
- Build the knowledge, capacity and strengths of those supporting participants
- Collaborate with a multidisciplinary team, including mentoring less experienced Behaviour Support Practitioners

- Current suitability at Proficient or Advanced level as a Behaviour Support Practitioner (documentary evidence required)
- Considerable experience providing support to people with behaviours of concern (professionally and/or personally)
- Flawless report writing skills

- Valid driver's licence and reliable motor vehicle
