About the Role

We are seeking a highly skilled and compassionate Client Outcome Facilitator to join our team at St Vincent de Paul. As a key member of our residential Alcohol & Other Drugs service, you will play a vital role in providing case management, advocacy, and support to clients engaged in our 12-week rehabilitation program.

Key Responsibilities
  • Provide holistic support to clients, connecting them with relevant resources and services to address related issues, including family and other relationships, emotional and physical health.
  • Develop and implement individualized case management plans, focusing on client needs and fostering client capacity toward independence.
  • Operate within the program Service Agreement, performing shift work, including afternoon, night, and weekend shifts.
  • Collaborate with a multidisciplinary team to ensure comprehensive support and care for clients.
  • Maintain accurate and confidential records, adhering to relevant policies and procedures.
Requirements
  • Minimum Certificate IV in Alcohol and Other Drug (AOD) or equivalent qualification.
  • Knowledge of and ability to work within relevant legislation and guidelines relating to the delivery of case management and safety and risk within the program area of operation.
  • Previous experience as a Support Worker or equivalent within the program area of operation.
  • Personal aptitude for working within set guidelines and adhering to all relevant policies and procedures.
  • Well-developed interpersonal, communication, and organizational skills, including the ability to exercise judgment and contribute to a team environment.
  • Ability to deliver advocacy on behalf of clients and ensure confidentiality of client information at all times.
  • Understanding of and commitment to the philosophy of the St Vincent de Paul Society Queensland and an ability to work in accordance with policies and procedures to promote the values and mission of the Society.
  • COVID-19 vaccination status is recommended for this role but not a mandatory requirement.
  • Valid Driver's License is required.
