Clinical Practice and Care Advisor

We are seeking a highly skilled and experienced Clinical Practice and Care Advisor to join our team at Claro Aged Care and Disability Services. As a key member of our Service Delivery Team, you will play a critical role in ensuring the delivery of high-quality, customer-focused healthcare services to our clients.
Key Responsibilities- Regional Subject Matter Expert: Provide clinical guidance and support to our Service Delivery Teams, ensuring the application of our Clinical Governance Framework, including Quality, Risk & Safety Management Systems.
- Clinical Care Planning: Develop and oversee the implementation of customer support plans and care with our Service Delivery Teams, ensuring that our clients receive the best possible care.
- Clinical Review and Assessment: Conduct regular reviews of our clients' clinical support needs in their homes and supported accommodation settings, identifying areas for improvement and implementing changes as needed.
- Stakeholder Engagement: Collaborate with stakeholders to support and upskill our care planning, risk assessment, and behavior support planning processes, ensuring compliance with regulatory requirements.
- Quality and Safety: Establish and foster relationships with our staff to ensure that quality is embedded within our support worker practice and all organizational programs and activities.
- Training and Development: Work with our Learning and Development Team to ensure that our training programs are clinically sound, reflecting the principles of independence, choice, and control, and person-centered support.
- Policy and Procedure Development: Support the development and review of our Clinical policies and procedures, ensuring that they are up-to-date and aligned with best practice.
- Current AHPRA Nursing Registration
- Significant Demonstrable Experience in providing clinical advice to Service Delivery Teams
- Well-Developed Knowledge and experience of the aged care or disability services sector
- Advanced Written, Verbal, and Negotiation Skills
- Advanced Interpersonal and Communication Skills
- Demonstrated Ability to work with a high level of independence and autonomy
- Well-Developed Time Management Skills ensuring stakeholder expectations are met
- Preparedness to work within our Philosophy, Values, and Mission
- Demonstrated Experience in Community and/or Rehabilitation Nursing
- Demonstrated Experience working within the Disability Sector
- National Police Records Check
- NDIS Worker Screen or Willingness to Obtain
- Evidence of Work Rights
- Preparedness to Undertake Intra and Interstate Travel
