
Mental Health Rehabilitation Specialist

In a supportive and therapeutic environment, you will work with individuals living with mental health conditions to help them recover and reintegrate into their community.
About the RoleThis specialist position is responsible for providing person-centred, psychosocial rehabilitation by identifying appropriate supports to help clients transition towards recovery, independent living and reintegration into their community.
- You will have experience in a similar role and be able to provide case planning and coordination support, advice and direction including high level problem solving to clients.
To be successful in this position, you will require tertiary qualifications in a social science or related field, or specialised skills sufficient to perform at this level. A current Driver's Licence, Satisfactory Criminal record check and Working with Children Check (NSW) are also required.
Key Responsibilities- Provide individualised support to clients, focusing on their unique needs and goals.
- Collaborate with other professionals to develop and implement client care plans.
- Conduct thorough assessments to identify areas of need and develop strategies for improvement.
- Monitor client progress and adjust support plans as needed.
- Develop and maintain relationships with community agencies and services to ensure seamless referrals and transitions.
