Care Partner Advocate

We are seeking a dedicated and compassionate individual to join our team as a Care Partner Advocate. This role involves establishing meaningful relationships with clients, understanding their needs, and providing personalized support to enhance their quality of life.
The successful candidate will have the opportunity to work closely with a diverse range of clients, each with unique experiences and challenges. As a Care Partner Advocate, you will be responsible for:
- Establishing rapport with clients and their representatives to build trust and understanding.
- Managing a caseload of clients, ensuring regular reviews and assessments to meet their evolving needs.
- Developing and implementing person-centered care plans that prioritize the client's autonomy, dignity, and well-being.
- Collaborating with multidisciplinary teams to ensure seamless delivery of care services.
To succeed in this role, you will require excellent communication and interpersonal skills, as well as the ability to empathize with clients from diverse backgrounds. You must also be able to work independently and as part of a team, prioritizing tasks and managing competing demands.
Requirements:
- Bachelor or equivalent qualification in a health-related field or allied health is preferred.
- Qualifications in Community Services or an equivalent field.
- Business acumen, budget handling, and spending, excel, and judgmental skills.
- Community aged care experience, including knowledge of issues facing older people or people with disabilities and their carers.
- Demonstrated understanding of Strengthened Aged Care Standards and best practice in community aged care.
- Excellent problem-solving, influencing, and negotiation skills, including capacity to facilitate complaint resolutions.
- Thorough understanding and ability to support compliance with WHS, Equal Opportunity, and other professional and legal requirements affecting day-to-day care and services for clients.
