We are seeking a dedicated and compassionate individual to join our team as a skilled Nursing professional. Our Continuity of Carer model allows for the development of strong relationships between carers and residents, providing a high level of personalized care.
The Role- You will be responsible for delivering proactive care that enhances the daily lives of our residents, ensuring their physical, emotional, and social needs are met.
- Develop and implement care plans tailored to meet the unique needs of each resident, promoting independence and empowering them to make informed decisions about their care.
- Ensure safe medication management, accurately documenting medication plans, interventions, and progress notes in accordance with relevant guidelines and regulations.
- A genuine passion for caring for others, with a commitment to delivering holistic care that supports residents' needs and promotes their well-being.
- Excellent communication skills, with the ability to effectively interact with residents, families, and healthcare professionals.
- Bachelor's Degree in Nursing or equivalent qualification.
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.
- The ability to quickly adapt to new information and systems, including the Aged Care Funding Instrument (ACFI).
- Experience working in aged care, hospitals, or community health settings, with a proven track record of delivering high-quality patient care.
- An understanding of the importance of accurate documentation and reporting, with the ability to maintain comprehensive records of resident care.
- Leadership and supervisory skills, with the ability to effectively delegate tasks and responsibilities to support staff.
