Clinical Care Manager Role

About the Role:

- Deliver excellent client care for elderly Australians located in the Northern suburbs of Perth.
- Manage and oversee high standards of care provided by our providers and staff.
- Utilize your clinical experience, customer service, and aged care management skills to make a mark on the community. This will help keep aging Australians safe at home for longer, contributing to maintaining our leadership in quality homecare.
- Develop, monitor, and reassess care plans in consultation with clients, their families, and other healthcare professionals.
- Collaborate with your Regional Manager to develop and shape our service offerings, ensuring they continue to be of the highest standard.
- Monitor care package budgets and ensure clients understand service fees.
- Manage Homecare packages programs.
- Engage with internal teams to facilitate client onboarding through the lead to conversion process.
- Influence and partner with schedulers, clinical teams, and support staff.
- Ensure compliance with all relevant legislative and industry standards.
- Proactively engage in continuous improvement across practice and service delivery.

Requirements:

- Hold a Health-related qualification such as an unrestricted current APHRA registration.
- Experience in aged care, with knowledge of Home Care Packages, Commonwealth Home Support Program, and Aged Care Quality Standards is desired.
- Frontline management skills.
- Current First Aid Certificate (HLTAID003/4) and current CPR certificate (HLTAID001).
- Right to work in Australia.
- A current driver's license, registration, and access to your own car with comprehensive car insurance.
- National Police Clearance or willingness to obtain (we will organize via WorkPro at no cost).
