Job Title:

Clinical Care Specialist

Job Description:
  • Provide direct patient care and collaborate with multidisciplinary teams to deliver exceptional healthcare outcomes.
  • Assess patient needs, develop tailored care plans, administer medications, and educate patients and their families on health management and well-being.
  • Work under the supervision of experienced clinical professionals, providing support in clinical procedures and ensuring seamless delivery of quality care services.
Requirements:
  • Current AHPRA registration as a Registered Nurse.
  • Minimum of 1 year of relevant experience in aged care or clinical nursing.
  • Qualifications: Bachelor's Degree or Diploma in Nursing (or equivalent qualification).
  • Other skills and requirements: Experience in palliative care, dementia care, and rehabilitation nursing. Certification in First Aid and CPR. Understanding of My Aged Care and NDIS frameworks. Minimum 2 years of work experience in aged care, geriatrics, or clinical nursing. Knowledge of aged care quality standards, policies, and regulatory compliance. Competence in medication administration, wound care, and chronic disease management. Excellent communication, leadership, and problem-solving skills. Ability to work independently and as part of a multidisciplinary team.
