About this Role:

  • As a Care Coordinator, you will be the key liaison between our clients, their families, and our care team.

We are seeking a dedicated and compassionate individual to join our home care support team as a Full Time Care Coordinator. In this role, you will ensure that our clients receive high-quality home care services by managing schedules, coordinating care plans, and providing exceptional support.

Key Responsibilities:

  • Matching clients with caregivers based on their needs and preferences
  • Scheduling and coordinating in-home care services
  • Communicating with families, caregivers, and healthcare professionals

  • Ensuring compliance with relevant standards and best practices
  • Providing exceptional customer service and client support

Required Skills and Qualifications:

  • A minimum of 2 years' experience in aged care or disability care
  • Knowledge of NDIS standards and Aged Care Standards
  • Strong organizational and communication skills
  • Ability to multitask and work in a fast-paced environment
  • Caring and compassionate attitude
  • Proficiency with scheduling software and basic computer skills
