About this Role:
- As a Care Coordinator, you will be the key liaison between our clients, their families, and our care team.
We are seeking a dedicated and compassionate individual to join our home care support team as a Full Time Care Coordinator. In this role, you will ensure that our clients receive high-quality home care services by managing schedules, coordinating care plans, and providing exceptional support.

Key Responsibilities:
- Matching clients with caregivers based on their needs and preferences
- Scheduling and coordinating in-home care services
- Communicating with families, caregivers, and healthcare professionals
- Monitoring and updating client care plans
- Ensuring compliance with relevant standards and best practices
- Providing exceptional customer service and client support
Required Skills and Qualifications:
- A minimum of 2 years' experience in aged care or disability care
- Knowledge of NDIS standards and Aged Care Standards
- Strong organizational and communication skills
- Ability to multitask and work in a fast-paced environment
- Caring and compassionate attitude
- Proficiency with scheduling software and basic computer skills
