Aboriginal Health Services Specialist

About the Role :

We are seeking a dedicated and compassionate Aboriginal Health Services Specialist to join our respected multidisciplinary team in Swan Hill.

As a key member of our team, you will collaborate with nurses, Aboriginal Health Workers, and community workers to provide culturally appropriate healthcare to Aboriginal and Torres Strait Islander individuals.

In this role, you will focus on ensuring a welcoming environment and delivering high-quality care tailored to the unique needs of our community.

Your responsibilities will include providing comprehensive case management for patients with complex and chronic conditions, conducting mental health consultations, and facilitating referrals as needed.

Additionally, you will provide GP services within the clinic and offer outreach services to the nearby town of Kerang one day per week.

There is also the opportunity to provide VMO services at Swan Hill District Hospital.

Key Responsibilities:
  • Deliver culturally sensitive healthcare services to Aboriginal and Torres Strait Islander clients in Swan Hill.
  • Manage complex and chronic conditions, conduct mental health assessments, and offer referrals.
  • Provide GP consultations within the clinic and outreach services to Kerang once a week.
  • Collaborate with a multidisciplinary team to ensure holistic care and support for all clients.
  • Participate in case management and contribute to the overall health and wellbeing of the community.

Essential Criteria:
  • Current AHPRA Registration (General or Specialist).
  • Experience or a strong interest in Aboriginal Health.
  • Compassionate and culturally aware approach to patient care.
  • Willingness to work as part of a collaborative team, contributing to positive community health outcomes.
  • Australian citizenship or Permanent Residency.
