Job Description

The Healthcare Coordinator for Older Adults plays a vital role in leading support and strengthening person-centred primary healthcare. This position is focused on implementing initiatives to improve the health outcomes of older adults.

This dual-focused role involves coordinating engagement and support for Allied Health professionals in Western NSW, as well as developing activities related to older people's health, aged care, and palliative care.

The position requires coordination with internal and external stakeholders to ensure successful planning and delivery of projects.

  • Implementing evidence-based programs to promote healthy ageing;
  • Coordinating engagement and support for Allied Health sector;

The role can be based in any one of our offices including Dubbo, Orange, Bathurst or Broken Hill.

The selected candidate will have a passion for promoting community wellbeing and improving the health outcomes of older adults.

Key responsibilities include:

  • Developing and coordinating activities related to older people's health, aged care, and palliative care;
  • Implementing ACOP in RACH;
  • Coordinating engagement and support for Allied Health sector;

The selected candidate will work collaboratively with internal and external stakeholders to ensure the successful planning and delivery of both projects.
