Care Coordination Role Overview

We are seeking a dedicated and experienced Care Coordinator to support the Registered Nurses and Care staff in delivering high-quality clinical care.

The Job

This role is responsible for monitoring care plans, providing guidance on complex nursing and care needs, and promoting continuous improvement in client care. You will work closely with the healthcare team to ensure that residents receive exceptional care and support.

Key Responsibilities:
  • Review and implement care plans to meet individual resident needs
  • Provide expert advice on complex nursing and care issues
  • Promote a culture of continuous quality improvement in care delivery
  • Maintain accurate and up-to-date documentation of resident care and progress
  • Liaise with healthcare professionals, doctors, and specialist service providers as required
Requirements:
  • 5 years' experience in aged care or health-related field
  • Registered Nurse qualifications
  • Strong leadership and interpersonal skills
  • Excellent written and verbal communication skills
  • Ability to work collaboratively as part of a multidisciplinary team
