Clinical Care Coordinator Job Description

The Clinical Care Coordinator is a key role within our healthcare organization, responsible for supporting Registered Nurses and Care staff in the delivery of clinical quality care. This position requires strong leadership skills, excellent communication abilities, and a commitment to providing person-centred care services.


Key Responsibilities:
  • Monitor and evaluate resident care plans to ensure optimal outcomes.
  • Provide guidance and support to nursing and care teams in responding to complex client needs.
  • Conduct regular audits and reviews to maintain high standards of care and promote continuous improvement.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination.
  • Document and maintain accurate records of resident care and progress.

Requirements:
  • Minimum 5 years' experience in caring for the aged or health-related field.
  • Registered Nurse qualification.
  • Strong leadership and interpersonal skills.
  • Excellent written and verbal communication abilities.
  • Ability to work collaboratively as part of a multidisciplinary team.
  • Numeracy skills and ability to maintain accurate records.
  • Sensitivity and understanding of the needs of elderly individuals in residential care settings.
