About the Role

Hammond Care is seeking an Expert Dementia Practitioner to join their team in Australia. This role involves delivering high-quality dementia behavior support services to individuals and their caregivers.

Key Responsibilities:

  • Conduct comprehensive assessments of individuals with dementia, focusing on biopsychosocial approaches.
  • Develop and implement innovative, non-pharmacological strategies to prevent or respond to changes in behavior related to dementia.
  • Collaborate with families, caregivers, and clinical staff to educate them on how dementia impacts each individual.
  • Equip carers and clinicians with skills to prevent and respond to behavior changes through modeling, coaching, and resource development.
  • Produce reports that provide contextualized recommendations for improving quality of life for individuals living with dementia.
  • Work closely with the broader Hammond Care team to build capability across the home and aged care sectors.

Skills and Qualifications:

  • Relevant qualifications in Allied Health, Nursing, Aged Care, or Mental Health are essential.
  • Experience in Australian residential aged care and/or community aged care is highly valued.
  • Strong communication and negotiation skills, with the ability to build relationships at all levels.
  • Excellent problem-solving and decision-making skills, with a focus on prioritizing tasks and meeting deadlines.
