About the Role
Clinical Nurse
The Clinical Nurse provides clinical and non-clinical home-based face to face assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool (IAT). The role involves working with clients and their representative/s to establish goals specific to the client's needs and develop appropriate strategies and actions, including referrals to aged care and non-aged care services. The goal is to provide comprehensive support to enable clients to achieve optimal health and wellbeing outcomes.

Key Responsibilities
- Provide high-quality, holistic assessments and care planning in accordance with organizational policies and procedures
- Collaborate with multidisciplinary teams to identify client needs and develop effective solutions
- Develop and maintain strong relationships with clients, families, and healthcare professionals to ensure continuity of care
- Participate in quality improvement initiatives to enhance assessment and care delivery processes

Requirements
- Mandatory qualifications: Nursing or Allied Health qualification; proof of registration (if applicable)
- Possession of a Class C driver's licence for travel requirements
- Ability to work independently and as part of a team to deliver client-centred care
