Job Description:

Clinical Case Coordinator

The Clinical Case Coordinator will support the Child and Youth Assessment & Therapeutic Service (CYATS) by collecting background information about clients, liaising with key stakeholders and managing administrative tasks.

The service aims to provide children and young people with neurodevelopmental vulnerability access to seamless assessment, treatment, management and support through a multidisciplinary team (MDT).

To be considered for this position, you will require:

  • A minimum of diploma qualified or relevant experience in a clinical case coordination role or similar
  • Demonstrated experience and skills in providing clinical case management support to vulnerable families that is child focussed, family centred and trauma-informed
  • Knowledge of child development, as well as neurodevelopmental disorders including FASD, ASD and Global Development Disorders

We are looking for someone who can work collaboratively with our team to provide responsive services to children and young people.
