Leadership Role Opportunity

Manage a team of staff, ensuring high-quality service delivery for the DSA program.

  • Job Description:
  • Oversee case management, manage resources, ensure compliance with KPIs, and maintain strong relationships with key stakeholders.
  • Apply your expertise in assessment and biopsychosocial approaches to prevent, minimize or respond to changes in behavior related to dementia.
Key Responsibilities
  • Engage with HammondCare's mission and practice guidelines, policies, and procedures.
  • Provide professional leadership for team members.
  • Allocate referrals to consultants in alignment with DSA KPIs and Activity Work Plan.
  • Maintain office environments, including safety, routine maintenance, and functioning equipment.
Required Skills and Qualifications
  • Tertiary qualifications in Nursing/Allied Health or health-related degree.
  • Experience in supporting older people and carers living with dementia, with emphasis on psycho-social approaches to case management.
  • Ability to manage and support a team across multiple sites and locations.
