Supported Living Facilitator

You will provide care to your clients living in the community, supporting their independence and enhancing their quality of life.

This role involves providing personal care, daily living assistance, including showering, personal hygiene, meal preparation, house cleaning, laundry and ironing services to clients within their homes.

  • Providing support with tasks such as showering, dressing, and grooming.
  • Assisting with household chores like cooking, cleaning and laundry.

You will also spend time interacting one-on-one with clients and their loved ones, providing emotional support and companionship. In-home visits will be made according to a schedule, requiring travel between locations using your own vehicle.

  • Must have access to a reliable vehicle for transportation purposes.

The working hours are primarily Monday to Friday, 9am-5pm. You will work collaboratively with colleagues to achieve the best possible outcomes for clients. A key aspect of this role is following each client's individualized Care Plan to ensure they receive tailored support.

  • Able to work effectively as part of a team to deliver high-quality care.
  • Effective communication skills in spoken and written English are essential.
  • Prior experience working in the Disability, Aged and Community sector is desirable but not mandatory - a positive attitude and willingness to learn are vital.
  • Hold a current NDIS Worker screening check.
  • Current CPR and First Aid Certificate required prior to commencing employment.
  • Holding an Open Driver's License and access to a vehicle is necessary.
  • Car Insurance must be in your name.
