Job Opportunity

We are seeking a dedicated and skilled professional to provide exceptional support services to our clients in their homes.

Key Responsibilities

  • To deliver high-quality support services under various funding sources, including private pay clients, and liaise with clients, family members, staff, and stakeholders to ensure services meet client needs, goals, and preferences.
  • To assess, plan, and review client plans as required, ensuring services are client-focused and consumer-directed with quality outcomes achieved using a flexible, responsive, creative, and collaborative approach.
  • To manage sales enquiries, develop and maintain new and existing referral sources, and build strong connections with clients and their families.
  • To assign staff to clients based on staff qualifications and availability, organise support to clients with disabilities, and monitor and report on their progress.
  • To refer clients to agencies that can provide additional help, assess client needs and resources for health, welfare, and other facilities and services, and maintain accurate records of incidents and feedback as required.
  • To reorganise work priorities to meet the needs of a fast-paced environment, support all clients to meet their goals, conduct Client Care Assessments and Care Planning, and supervise and manage Care Staff.
  • To discuss and promote Right at Home's services with medical professionals and referral sources, report to head office, and work in a fast-paced environment.

Essential Skills

  • Experience in assessing and planning client needs.
  • Sound understanding of Aged Care and Disability systems.
  • A strong client focus based on their goals, preferences, and needs.
  • Demonstrated experience managing staff.
  • Current National Police Check, Working with Children Check, Full Driver's Licence, Organisational, administrative, and time management skills.
