We are seeking a highly motivated and dedicated individual to join our Fostering and Permanency team as a Case Manager. As a Case Manager, you will play a critical role in supporting children and young people in care, as well as their carer families.
Key Responsibilities- Case Management
- Maintain accurate and up-to-date client records and case management systems, adhering to policies, procedures, standards, and legislation.
- Respond promptly to correspondence and attend supervision meetings as required by management.
- Participate in ongoing training and development opportunities to enhance your skills and knowledge.
- Casework
- Stay current with the framework governing our Out-of-Home Care (OOHC) practice, including internal policies, procedures, NSW Child Safe Standards, and relevant legislation.
- Advocate on behalf of clients, promoting their best interests at all times.
- Provide support to children and young people in care, as well as their carer families, through home visits, trauma-informed care education, behavior support strategies, advocacy, and referrals to relevant therapeutic services.
- Develop and implement effective case plans, leaving care plans, and aftercare plans with clients, carers, birth families, and relevant professionals within required timeframes.
- Conduct case reviews to ensure case plan goals are being addressed or updated as required.
- Develop and ensure implementation of effective behavior management and support plans within required timeframes.
- Monitor, implement, and collect data on behavior management strategies.
- Meet regularly with clients and their carer families.
- Arrange birth family contact in accordance with court orders, care plans, and the best interests of children and young people in care.
- Maintain a good understanding of the quality of care received by children and young people in care.
- Prepare high-quality organizational reports in a timely manner.
- Prepare court materials (e.g., S82 reports and affidavits) to a high standard and in a timely manner.
- Collaborate with Community Services in accordance with the Case Management Policy and case-specific needs.
- Liaise with the Case Manager regarding issues beyond your delegation.
- Liaise regularly with carer families to promote positive and high-quality placements.
- Liaise with birth families to promote their engagement in meeting children and young people's needs and promoting their birth family identity.
- Liaise with other service providers to promote the needs of children and young people in care.
- Work from a strengths-based and child-focused perspective at all times.
- Respond promptly to any concerns raised about the safety, welfare, and well-being of children and young people in care.
- Workplace Health and Safety
- Prepare incident reports to a high standard and in a timely manner.
- Advise the Case Manager of any grievances, critical incidents, issues, or concerns regarding clients, employees, and foster carers in a timely manner.
- Promotion of Allambi Care Values
- Participate as an active team member within Allambi Care.
- Present yourself as a positive team member of Allambi Care at all times.
- Attend team meetings.
- Participate in Allambi Care foster care events.
- Participate in interagency events as a willing Allambi Care team member when requested by management.
- Participate in local networking meetings to ensure Allambi Care is aware of the expectations of the wider OOHC sector.
- Ensure all casework tasks are performed satisfactorily to allow Allambi Care to meet its requirements to service users, funding bodies, and the Board.
- Minimum qualification of a Diploma in Community Services or a social work, social science, welfare, or a related discipline.
- Experience in case management and/or working with families or children and young people.
- Demonstrated understanding of the needs and issues facing OOHC stakeholders.
- Demonstrated excellent verbal and written communication skills.
- Experience in Microsoft Office Applications.
- Knowledge of Needs-Based Framework, TCI concepts, and strategies.
- Full-time employment.
- Four weeks of annual leave per year.
