  • Provide personalized support to clients in the community and at home, assisting with daily living tasks and promoting independence.
  • Accompany clients to appointments, social activities, and community programs, fostering connections and a sense of belonging.
  • Maintain accurate records and documentation as required, ensuring seamless communication between team members and stakeholders.
  • Follow established care plans and company policies to guarantee client safety and quality service delivery.
Requirements
  • Previous experience in community support work or a related field is essential.
  • A current driver's license and reliable vehicle are necessary for transporting clients.
  • Strong communication and interpersonal skills are crucial for building effective relationships with clients, families, and colleagues.
  • Flexibility to work varied hours, including weekends if required, is a must.
  • A valid Working With Children Check and Police Check (or willingness to obtain) is also required.
  • Relevant qualifications, such as Certificate III/IV in Individual Support, Community Services, or equivalent, are highly regarded but not essential.
