Key Responsibilities- Provide dietetic services to patients in the CALHN Integrated Care setting, working collaboratively with multidisciplinary teams to achieve optimal outcomes.
- Develop and implement dietetic plans, taking into account the complex needs of patients with multiple health conditions.
- Collaborate with health, social, and other relevant services to ensure seamless care coordination and patient-centered approaches.
- Participate in departmental educational, quality improvement, and planning activities to enhance professional knowledge and skills.
- Provide professional/clinical supervision to junior Dietitians, Allied Health Assistants, and students, as required.
