Job Title: Community Health Worker

The role of the Community Health Worker is a vital part in providing health services to marginalised and vulnerable adults in Queensland.

Key Responsibilities:

  1. Manage client cases, assess need and risk, and deliver outreach services to hard-to-reach groups.
  2. Contribute to multi-disciplinary comprehensive assessments to establish eligibility for health and social care services.
  3. Undertake continuous risk assessment and contribute to effective risk management.
  4. Ensure that the care provided respects equality and diversity, is supportive of service users and their carers.
  5. Assess, plan, and determine the care needs of service users using agreed frameworks and guidance.
  6. Participate in and contribute to ongoing quality of service and care, identifying and reporting any risk issues or other significant factors.
  7. Recognise and respond appropriately to challenging behaviour presented by service users or others.
  8. Identify safeguarding issues for both children and vulnerable adults and implement trust procedures as required.

Requirements:

  • Registered Nurse with experience working with marginalised and vulnerable populations.
  • Knowledge and understanding of mental health legislation and policies.
  • Ability to communicate effectively with clients, colleagues, and external agencies.
  • Strong problem-solving and analytical skills.
  • Excellent interpersonal and organisational skills.
