Job Title: Palliative Care Social Worker

Role Summary:

We are seeking a skilled and compassionate Palliative Care Social Worker to join our multidisciplinary team at Westmead Hospital. The successful candidate will be responsible for delivering high-quality care to patients and their families in the Western Sydney Local Health District.

The role offers opportunities to develop skills and expertise in providing palliative care services, including working with culturally and linguistically diverse client groups.

  • Main Responsibilities:
  • Providing counselling and case management to patients and their families
  • Working collaboratively with multidisciplinary teams to achieve patient goals
  • Developing and implementing individualized plans to meet patient needs

To be successful in this role, you will need:

  • Essential Qualifications:
  • Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW)
  • Demonstrated understanding of psychosocial issues associated with acute and chronic illness
