The role of Health Services Lead is a key position within the Primary Health Care leadership team. The successful candidate will be responsible for advancing priorities through supporting the ongoing development and implementation of assigned Primary Health Care, Wellness, Chronic Disease Management, Department of Family Practice programs services and initiatives.
">- Develop and implement strategies to advance patient-centered, community-based primary health care programs and services.
- Manage day-to-day operations of chronic disease teams with a focus on Diabetes Centres and the wellness portfolio across Eastern Zone, spanning multiple sites.
- Facilitate the development and implementation of consistent Primary Health Care, Chronic Disease management and organizational priorities while respecting uniqueness and individuality of teams and communities.
The ideal candidate will possess a baccalaureate degree from a health discipline or related field, along with a minimum of three to five years' recent and related progressive leadership experience in a healthcare setting. Experience in chronic disease management and primary health care, specifically diabetes management, will be considered an asset. Additionally, the ability to work collaboratively with various professional and community individuals, demonstrating high-level judgement, tact, creativity, diplomacy, discretion, and confidentiality will be essential.

- A competitive hourly rate between $40.95 and $54.42.
