As a skilled and compassionate professional, you will play a vital role in the delivery of holistic care to older adults experiencing mental health challenges, chronic conditions, and complex needs. Our team is seeking a dedicated nurse to provide early intervention, health education, and recovery-focused support within clients' homes and community settings.

Job Summary

This is an exciting opportunity to join our multidisciplinary team and make a real difference in the lives of older people. As a Community Nurse, you will work closely with clients, carers, and other healthcare professionals to deliver high-quality care and improve outcomes.

About the Role:

  • Provide direct clinical nursing care to clients in their homes, adhering to best practice nursing procedures
  • Conduct thorough clinical assessments, dressings, medication reviews, and catheter changes as required
  • Develop comprehensive nursing care plans in collaboration with clients, carers, and other healthcare professionals
  • Liaise with all parties involved in client care to ensure holistic health and wellbeing

Requirements:

  • A genuine passion for delivering high-quality care to the elderly
  • Current registration with AHPRA following completion of a Bachelor of Nursing or similar qualification
  • Minimum 3 years post-graduate nursing experience, with a focus on clinical assessments and community care
  • Demonstrated ability to work effectively in a team environment
