Community Nursing Role

This position is an exciting opportunity to join our team of healthcare professionals in delivering high-quality care to patients in their homes.

As a community nurse, you will work autonomously and collaboratively with other clinicians to develop responsive strategies to meet the changing health needs of the community.

  • You will be responsible for providing home visits throughout the community, primarily working between the hours of 8am to 4:30pm, Monday to Friday with rotating weekends.
Key Responsibilities
  • Liaising with other health professionals to facilitate case management and care planning
  • Developing responsive strategies to meet the changing health needs of the community
  • Providing high-quality care to patients in their homes
Required Skills and Qualifications
  • A current registration as a Registered Nurse (Division 1) with AHPRA
  • A valid Working with Children Check
  • Recent nursing experience in Community Nursing or a demonstrated interest in Community Nursing
