Job Summary
This position provides a multi-disciplinary approach to the screening, assessment, and care planning of older people (generally aged over 70) admitted to the Emergency Departments to ensure appropriate and coordinated assessment and care for the designated group in the Emergency Department.
Key Responsibilities
- Provide advanced skills in the assessment and care management of older people with complex needs, including cognitive and behavioral assessment.
- Communicate clearly and succinctly both verbally and in writing, and work effectively and collaboratively within a multi-disciplinary team.
- Prioritize and manage own caseload, and demonstrate competence in the use of electronic information systems.
- Collaborate with healthcare professionals, including doctors, allied health, and community services, to ensure comprehensive care for older patients.
- Develop and implement individualized care plans that address the unique needs of older patients.
Requirements
- Registration with the NSW Nurse's Registration Board.
- Relevant post-registration qualification and at least 3 years' experience working in the clinical area of their specified post-graduate qualification.
- Demonstrated advanced skills in the assessment and care management of older people with complex needs.
- Demonstrated high level ability to communicate clearly and succinctly both verbally and in writing.
- Demonstrated ability to work effectively and collaboratively within a multi-disciplinary team.
- Demonstrated competence in the use of electronic information systems.
- Current NSW Class C driver's licence.
